Verification status does not constitute an endorsement, guarantee of service quality, or legal advice. You use this information at your own risk.

4. User Submissions

Applications and requests must contain accurate information. We reserve the right to reject or remove submissions that violate these terms.

5. Intellectual Property

ProviderCheck content, branding, and methodology are protected. Trust Mark badges may only be used in accordance with our badge usage guidelines.

6. Limitation of Liability

To the maximum extent permitted by law, ProviderCheck shall not be liable for any indirect, incidental, or consequential damages arising from use of the service.

7. Verification Terms

• Verification is point-in-time and reflects evidence available as of the displayed verification date.
• Verification does not guarantee future conduct, service quality, or uninterrupted compliance.
• Payment does not guarantee approval. PAYMENT ≠ VERIFIED.
• The Trust Seal may be suspended or revoked when verification status changes.
• Providers must not alter, forge, or misrepresent the Trust Seal or verification status.
• ProviderCheck may require re-verification at any time according to policy.
